DOL Releases New Model FMLA Forms

The Department of Labor (DOL) published new model FMLA forms on July 16, 2020.

The new forms are designed to be easier to understand for employers, leave administrators, healthcare providers, and employees seeking leave. However, the information collected is the same information, so it may be collected in any format.

HR Services has modified the FMLA forms available in the HR Library, search for Leaves for more information on the DOL changes. The new DOL FMLA forms do not have any applicability to the Families First Coronavirus Response Act (FFCRA).
