DOL: FMLA Forms

The U.S. Department of Labor (DOL), Wage and Hour Division addresses the expiration date of the Family and Medical Leave Act (FMLA) forms available on its website.

The expiration date of the optional-use forms provided by the DOL has passed, but the information is still applicable. The expiration date on the DOL forms is related to the collection of information as required by the Office of Management and Budget (OMB), and not relevant to the content of the required information.
