Plan Highlights

The following are highlights under the four coverage areas of the Supplemental Employee Benefits Program.

Group Term Life Insurance Plan

  • Plan covers more than 50,000 employees in 260 districts. Coverage provided by Lincoln Financial Group, an A+ A.M. Best rated company.
  • Rate is $.10/$1,000 of coverage for Basic Life and AD&D. Minimum is $10,000/employee or $1 per employee per month for $10,000 coverage. Basic Life and AD&D is employer-paid.
  • Guaranteed issue for up to $100,000 of Basic Life and AD&D. No underwriting for groups with fewer than 1,000 employees. Districts with more than 1,000 employees will be individually underwritten.
  • Employee-paid Optional Term Life can be purchased up to $100,000, guaranteed issue. Employee maximum of $500,000 (not to exceed five times annual salary). Optional Dependent Life is also available for spouses and children.
  • Rates are guaranteed to September 1, 2012.

Group Long-Term Disability Plan

  • Available to all active, non-seasonal employees who work a minimum of 20 hours per week. All eligible employees must participate.
  • Plan underwritten by Lincoln Financial Group.
  • Rate is $5 per employee per month for groups with fewer than 500 employees. Districts with more than 500 employees will be individually underwritten. Rate is guaranteed until September 1, 2012, employer-paid.
  • Benefit percentage is 60 percent, maximum monthly benefit of $5,000 with a 90-day elimination period.

Group Automobile/Homeowner's Plan

  • Employer-sponsored, employee-paid, voluntary plan that provides group discounts for automobile and homeowner's insurance underwritten by Liberty Mutual.
  • Other coverage is available for personal liability, seasonal and rental properties, motor homes and recreational vehicles, boats, scheduled property, and identity fraud expense.
  • Streamlined underwriting process, except those in high-risk, hurricane-prone areas.
  • Customized communication material, enrollment handled completely by Liberty Mutual. Payroll deduction, checking account withdrawal, or employee direct bill available.

Section 125 Cafeteria Plan Administration (Medical Expense and Dependent Care Reimbursement Accounts)

  • Plan administration provided by First Financial Administrators, Inc.
  • No administrative or implementation fee for plan administration.
  • Debit cards for medical expense reimbursement accounts provided at no fee.
  • Direct deposit of flexible spending account reimbursements.
  • Upfront money to reimburse medical expenses with no risk to district.
  • Online employee access for claim review and reimbursement history.
  • Toll-free fax for claims submission and all claims processed in-house with no minimum amount for reimbursement checks.
  • IRS compliance and annual nondiscrimination testing ensured.
  • Access to more than 100 highly rated insurance carriers to create a unique customized benefit plan.
  • Ability to consolidate online monthly billing and upload to district payroll system.
  • No requirement to purchase products with potential tax savings.
