Entry Requirements

Detailed criteria and rules are covered in the brochure and posted on line. Questions on the following pages should be answered by the school board and submitted with a resolution indicating the board’s decision to nominate the superintendent. The completed entry form (signed by the school board president), resolution, and a resume (no more than three pages) should be sent to the executive director of the district’s respective education service center (ESC) by May 17, 2010. Responses must be limited to space available on the official entry form. Any additional materials submitted—other than the resolution, resume, and superintendent’s photo on a separate sheet—will not be accepted. Entries must be either postmarked by May 17 or delivered to the ESC by close of business that day. Please note that board action is required for the resolution. Participating districts should place an item to cover this on their next board meeting agendas.

If you recreate the entry, include headings in the same order and keep the same format as the enclosed originals (i.e., complete one heading per page, keep answers to one page—one side only—for each heading, number the pages). Use 12-point Times New Roman, 1.5 spacing, and
1-inch margins
. Save the text as a Microsoft Word file and you are ready to go. Please do not submit a spiral-bound entry.

To save space on the final entry submitted to the ESC, you can delete questions under each major category. Please retain headings and page numbers for clarity.
